What happened: Thousands of Arizonans were affected by a data breach earlier this year that targeted the state’s Medicaid agency.

How did it happen: An internal review found that a programming error led to the mix-up. ​The forms do not contain social security numbers but do have names and dates of birth.

Outcome: the agency is informing those impacted by the breach on Monday. Anyone who was affected was advised to place a free credit report fraud alert or a credit alert security breach on their account, request a credit report or report any suspected identity theft.
